Training Requisition

If you are already in possession of a unit or an array of units, please submit the following information to your designated point of contact. If you do not remember your point of contact, or if you do not have a unit, please indicate this clearly in the first line of an email to:

Upon receiving your application, you will be issued a:

  • Requisition Number: Unique identifier for tracking the request.
  • Date: Date the requisition is received.
  • POC: Point-of-contact within Douglas Mining to facilitate the structuring and deployment of one of our field team.

You must submit all or part of:

Requestor Information

  • Name of the person or department requesting the training.
  • Contact information (phone number, email).
  • Date of the request.
  • Title or name of the training program.

Training Schedule

  • Proposed date(s) and time(s) for the training.
  • Duration of the training (e.g., half-day, full-day, multiple sessions).
  • Flexibility in scheduling (e.g., alternative dates or times).

Training Location

  • Preferred venue for the training (e.g., on-site, off-site, online).
  • Any special requirements for the location (e.g., room setup, equipment needs).

Required Resources

  • Materials requested for the training (e.g., manuals, online resources).
  • Equipment requirements (e.g., computers, projectors, internet access).
  • Support services (e.g., IT support, catering).

Cost and Budget

  • Estimated cost of the training program.
  • Budget allocation or source of funding.
  • Approval for expenditure, if required.

Approval and Authorization

  • Details of the approval or requestor and any necessary approval and the dates of that approval  (e.g., department head, HR manager).
  • Any additional authorization at the time of training required by organizational policy such as waivers, badges, or passes.

Additional or Special Instructions

  • Post-training reporting requests (e.g., submission of certificates, training completion reports).
  • Any other relevant information that may affect the training (e.g., prerequisites, special accommodations).

Participant Details (if known)

  • List of participants, including names and roles.
  • Any specific needs or considerations for individual participants (e.g., movement restrictions, accessibility needs).
